Enrollment

If you’re application has been accepted, a Family Advocate will contact you to schedule an enrollment appointment.  Please bring the following documentation along with you to your appointment (The following information pertains to the child being enrolled and the parents/gaurdians):

• Social Security numbers

•  Immunization record of child

• Child’s latest physical (MUST be within the past 12 months)

• Verification of birth of child (Birth Certificate or Hospital Certificate)

• Proof of income

• Dental exam performed by a licensed dentist MUST be completed within first 90 days of child’s enrollment
